Systemic Local Anesthetics for Acute and Chronic Pain Local anesthetics and related sodium channel blockers such as mexiletine can be administered as systemic analgesics as well as for local anesthesia. The effect is worse with rapid injection, and is more pronounced in elderly patients, especially those with reduced intravascular fluid volume (also see Chapter 35). The data are less consistent on the effect of other factors, such as age of onset, number of seizures in 24 hours, duration of seizure, type of seizure, family history, and prior history of febrile seizures.
However, long-term seizure control in patients with chronic epilepsy can be obtained, with skilful treatment, in about 30%, and it should be further possible to greatly lessen the frequency or severity of seizures in many of those in whom complete control is not possible. The ultrasound transducer is placed medial to the coracoid process in a parasagittal plane (about halfway between supraclavicular and axillary regions). It is an option for patients who can tolerate more calories, have a large appetite, or who cannot accept the restrictions of the classical ketogenic diet. Hydrochloric acid, a strong acid, has a pKa of -7, whereas carbonic acid, a weak acid, has a pKa of 6. This rigidity can even make ventilation via a bag and mask during induction of anesthesia nearly impossible because of vocal cord rigidity and closure. The nature and the duration of surgery must be understood so that the epidural may be inserted at the appropriate level (Table 17. The material-specific model of memory function has been a dominant concept in the neuropsychological understanding of memory function for over 50 years (15).
